This seminar will provide an introduction to Advance Directives and the new Illinois Physician Order for Life Sustaining Treatment (POLST). Our panelists will begin with an overview of the common Illinois Advance Directives, including the 2011 revisions to the Power of Attorney Act.

The implementation of the new Illinois Physician Order for Life Sustaining Treatment (POLST) will also be discussed. The statute directing the State of Illinois to create a POLST Paradigm was enacted in January, 2012. In the year since the enacted of the statute, great strides have taken place in the creation of the POLST form and implementation of POLST.

Finally, our panelists will discuss the interaction between the various forms and how they may work together to provide adults, and, in particular, the aging population, with the ability to appoint decision makers and set their End of Life affairs in order and to allow themselves to die with dignity.
